How we help clients with Congestive Heart Failure (CHF)

When Heart failure occurs, oxygenated blood enters the left side of the heart from the lungs, but the ventricle cannot pump hard enough to push all the blood out of the body, so some of the blood is left behind. The blood backs up, causing fluid to leak back into the lungs. This is known as "Left-sided Heart Failure"(it can take place on the right-side too "Right-sided Heart Failure").
The main symptoms of Congestive Heart Failure are:
Fatigue
Sudden Weight Gain
Shortness of Breath
Edema
How "KeynCare" Helps Clients Who Have CHF?
Weigh our clients according to the care plan.
Gather accurate and timely blood pressure readings and report them to our health care team for medication adjustments.
People with CHF who take in more fluids than they excrete will develop edema. Because fluid balance is of high importance for CHF clients, we record fluid I & O amounts.
Document when clients are adding salt to their food, take over the counter medications, or not taking their prescribed medications.
Document activities that seem to bring on, or worsen a clients fatigue. Then take steps to modify or avoid those type of activities if possible.
Record clients fatigue on a rate scale and report data to health care team.
